It's happened at times, mostly small things with a few big exceptions like this thing, but it was usually caught way before a game went on sale or went live because it was part of a job to make sure this kind of thing never happened.

The ones I can think of are Terraria's initial launch had Bartz as the player character sprite because they had to launch way early due to a leak, and there was one in warframe where a texture that ended up in the game that said "this is a{other game} texture, Do not ship" all over. You know, funny

The last time I, personally, witnessed an "unintentional" placeholder texture i was playing the MMO Runes of Magic and my friends and I discovered where the next expansion's zone will be, clipped OoB, & got in months early into the map lol

Now there's an AI placeholder every month, but, y'know, nbd

the only explanation other than the obvious "they're lying" that makes sense is that the AI art isn't as clearly something that needs to be removed when they glance through stuff as their previous 'just plain bad' textures were. the thing is it SHOULD be clear so you notice and remove it!

I sometimes suspect this has to do with placating the kind of execs who don't know how to look at unfinished work.

The kind of people who see a loose sketch or untextured model and just go "It's sloppy and ugly change everything." Strong incentives to gussy everything up for them.

Another problem is when you're having to say "OH THAT, AI, WAS JUST MEANT TO BE A PLACEHOLDER" instead, because your AI is being fed stolen content that still has (unused) placeholder assets in the files, and decides to "helpfully" find use for those (intentionally-removed) assets.

This rule is for anything with a UI.

Its just the game dev equivalent of those people who go "oops, that text totally wasn't for you, it was for this other person I'm texting, just ignore it" when they send you a series of texts saying how they want to sleep with you followed by a dick pic and you get mad at them for sending it
