I think that people are used to the way that things are right now. The way that they've seamlessly fit into the fact that he's in desperate measures and they're very used to the whole situation makes me think it's been going on for quite some time. He's not well and hasn't been for a while.

The edema in his feet and legs is going to make it difficult and painful to walk. Compression would help but he's not wearing any. Diuretic would help but he's likely not taking one because he thinks aspirin is a cure all, but it would explain why he appears to be wearing adult diapers sometimes

Whatever is going on with him is very serious and it's very very bad for him and it's not something that he's ever going to heal from. I think there are a multitude of issues going on personally. He's a guy who never took care of himself and he's already 79. Not much longer left at this point.
